We're growing our rehab team and looking for a full-time Physical Therapist who wants to practice differently — one who values specialized dementia rehabilitation, creative clinical problem-solving, and person-centered, function-focused therapy. Applicants wanting part-time are also encouraged to apply!

Responsibilities

• Assess resident needs and develop individualized treatment plans

• Implement and modify treatment plans to address physical, cognitive, and emotional needs

• Apply function-focused, person-centered approaches to support independence and mobility

• Collaborate with the interdisciplinary team to ensure coordinated, relationship-based care

• Educate residents and families on therapeutic exercises and techniques

• Monitor and report on resident progress and outcomes

• Maintain accurate, up-to-date patient care records
