Job Overview:

Join our compassionate team as a Certified Home Health Aide and make a difference every day! Help our clients achieve personal goals, maintain independence, and integrate fully into their communities through personalized, one-on-one care.

Key Responsibilities:

• Assist with daily living activities such as personal hygiene, meal preparation, and mobility

• Follow Individualized Care Plans (ICPs) tailored to each client's needs

• Provide companionship, emotional support, and social engagement

• Assist with medication reminders and monitor overall well-being

• Support clients with transportation to appointments and community outings

• Maintain accurate care documentation and follow care guidelines

Qualifications:

• High School Diploma or GED

• At least 18 years of age

• Prior home health aide experience preferred

• Valid Driver's License and reliable transportation

• CPR and First Aid certification

• Certified HHA

• PPD result
