About the Role:

As an Occupational Therapist (Per Diem) at Terraces of Lake Worth Care Center, you will have the opportunity to work with a diverse range of residents, assessing their physical and cognitive abilities, and developing individualized treatment plans to enhance their overall well-being. Your expertise will enable our residents to maintain their independence, safely navigate their environment, and participate in activities they enjoy.

Responsibilities:

• Conduct thorough assessments of residents' physical and cognitive abilities

• Develop and implement individualized treatment plans to address residents' goals and needs

• Provide occupational therapy interventions, including but not limited to, range of motion exercises, strengthening and stretching techniques, and cognition and memory exercises

• Collaborate with other healthcare professionals to ensure comprehensive care

• Maintain accurate records of treatment plans, progress notes, and discharge summaries

• Participate in interdisciplinary team meetings to discuss resident progress and needs
