Medical Records Specialist – Long-Term Care (Preferred Experience)

The Mariemont Care Center is seeking a detail-oriented and dependable Medical Records Specialist to join our long-term care team. This role is essential in maintaining accurate, timely, and compliant resident records while supporting smooth clinical operations across the facility.

Responsibilities

• Maintain and organize resident medical records in compliance with state and federal regulations

• Ensure charts are complete, accurate, and up to date

• Process admissions, discharges, and record transfers

• Manage documentation requests, audits, and chart corrections

• Coordinate with nursing, admissions, and administrative teams

• Support MDS, billing, and clinical departments as needed

• Maintain confidentiality in accordance with HIPAA standards

• Assist with quality assurance and regulatory readiness

Qualifications

• Previous experience in medical records in a long-term care (LTC) facility strongly preferred

• Knowledge of skilled nursing documentation and compliance standards

• Strong attention to detail and organizational skills

• Ability to manage deadlines and prioritize workload

• Proficient with electronic medical records (EMR) systems preferred

• Strong communication and teamwork skills
