Social Worker Responsibilities:

• Collaborate with multidisciplinary teams to ensure comprehensive care

• Coordinate social services, including discharge planning and community referrals

Social Worker Requirements:

• Bachelor's degree in Social Work or related field

• Prior experience in social work or a related field preferred
