Deer Meadows Home Health is growing, and we're looking for compassionate, motivated Occupational Therapists (OTR/L) to join our Home Health and Community-Based Therapy Team!

Our therapists serve patients in their homes, assisted living communities, independent living residences, and personal care settings throughout Northeast Philadelphia and Bucks County.

Compensation & Incentives

We value the expertise and dedication of our therapy professionals and offer a competitive compensation structure!

Full-Time Occupational Therapists:

• Up to $110,000 annually, based on experience and qualifications

• $5,000 Sign-On Bonus for full-time employees!

Per-Visit Compensation:

• Start of Care (SOC): $175 per visit

• Resumption of Care (ROC): $100–$125 per visit

• Recertifications: $100–$125 per visit

• OASIS Discharge: $100–$125 per visit

• Routine OT Visits: $80–$90 per visit

• Meetings: $40 per meeting

Mileage Reimbursement:

• $0.58 per mile, reimbursed after the first 20 miles of the workday

Compensation is based on experience, qualifications, employment status, and applicable visit type.

What You'll Do

• Evaluate patients and develop individualized occupational therapy treatment plans

• Provide skilled therapy interventions focused on activities of daily living (ADLs), independence, safety, and functional mobility

• Deliver one-on-one care in patients' homes and community-based settings, including ALF, IL, and Personal Care

• Educate patients and caregivers on adaptive equipment, energy conservation, and home safety

• Collaborate with nursing, physical therapy, speech therapy, and other interdisciplinary team members

• Complete accurate and timely documentation using Home Care Home Base (HCHB EMR)

• Independently manage your caseload and daily schedule while maintaining excellent patient care

What We're Looking For

• Graduate of an accredited Occupational Therapy program

• Active Pennsylvania Occupational Therapist license (OTR/L) or eligibility for licensure

• Home Health experience preferred

• Familiarity with Home Care Home Base (HCHB) is a plus

• Strong clinical judgment, communication, and organizational skills

• Ability to work independently in community-based settings

• Valid driver's license and reliable transportation
