You will assess resident needs and develop individualized treatment plans to address physical, cognitive, and emotional needs.

Responsibilities:

• Assess resident needs and develop individualized treatment plans to achieve goals
• Implement and modify treatment plans to address physical, cognitive, and emotional needs
• Collaborate with interdisciplinary team to ensure coordination of care
• Educate residents and their families on therapeutic exercises and techniques
• Monitor and report on resident progress and outcomes
