Director of Social Work

Salary: Up to $95,000 per year

Sans Souci Rehabilitation and Nursing Center is seeking an experienced and compassionate Director of Social Work to lead our Social Services Department. This is an excellent opportunity for a dedicated professional who is passionate about resident care, advocacy, and making a meaningful difference in the lives of our residents and their families.

Key Responsibilities:

• Oversee and manage the daily operations of the Social Services Department.

• Ensure residents receive appropriate psychosocial support, counseling, and assistance with adjustment to the facility.

• Coordinate admissions, discharges, and transition planning in collaboration with the interdisciplinary team.

• Serve as a liaison between residents, families, facility staff, and outside agencies.

• Address resident and family concerns, grievances, and requests in a timely and professional manner.

• Ensure compliance with all applicable federal, state, and regulatory requirements.

• Maintain accurate and timely documentation in accordance with facility policies and regulatory standards.

• Supervise, mentor, and support Social Services staff.

• Participate in care plan meetings and interdisciplinary team discussions.

• Advocate for residents' rights, dignity, and quality of life.

Qualifications:

• Bachelor's degree in Social Work or a related field required; Master's degree preferred.

• Prior experience in a skilled nursing facility or long-term care setting strongly preferred.

• Previous supervisory or departmental leadership experience preferred.

• Strong knowledge of discharge planning, psychosocial assessments, and long-term care regulations.

• Excellent communication, organizational, and interpersonal skills.

• Ability to work effectively with residents, families, and an interdisciplinary healthcare team.
