Fifth Star Care Management is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to join our IN-HOUSE therapy team! New Grads Welcome!

Position Summary

We are looking for an SLP who is passionate about providing high-quality, resident-centered care and helping patients improve their communication, cognition, swallowing, and overall quality of life.

Responsibilities

• Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders

• Deliver evidence-based therapy services in accordance with physician orders and facility policies

• Collaborate closely with an interdisciplinary team, including Physical Therapists, Occupational Therapists, Nurses, and Physicians, to ensure comprehensive patient care

• Educate patients, families, and caregivers on treatment goals, home programs, and progress

• Maintain accurate and timely clinical documentation in compliance with state, federal, and facility guidelines

• Participate in care plan meetings and contribute to discharge planning as needed

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program

• Current State Licensure or eligibility for licensure as a Speech-Language Pathologist

• Current Certificate of Clinical Competence (CCC-SLP) from ASHA

• CFY candidates welcome to apply

• Prior experience in a Skilled Nursing Facility or similar setting preferred, but not required

• Strong interpersonal, organizational, and communication skills

• Compassionate, dependable, and patient-focused approach to care
