About the Role:

As a Certified Nursing Assistant Home Health Aid, you will:

• Provide personalized care services to clients in the comfort of their own homes, including assistance with activities of daily living (ADLs), personal care, and homemaking tasks

• Monitor and report changes in clients' conditions to registered nurses and other healthcare professionals

• Collaborate with healthcare teams to develop and implement individualized care plans

• Maintain accurate and up-to-date records of client care services and progress

• Provide emotional support and companionship to clients and their families

Responsibilities:

• Deliver high-quality, patient-centered care to clients

• Demonstrate strong communication and interpersonal skills

• Work effectively as part of a multidisciplinary care team

• Maintain confidentiality and uphold professional boundaries

• Participate in ongoing training and education to stay current with industry best practices

Requirements:

• Current certification as a Certified Nursing Assistant (CNA) in the state of Louisiana

• High school diploma or equivalent

• At least 3 years of experience in home health care or a related field

• Ability to travel to client homes within the Baton Rouge, LA area
