Schedule: Full-Time or Part-Time | Flexible | No Nights or Weekends

Compensation:

• MD/DO: $180,000 to $220,000 annually, based on credentials, experience, and schedule

• NP: Competitive Compensation, based on experience and level of autonomy

About the Role

Medley Health is seeking a compassionate, motivated Physician or Nurse Practitioner to join our growing post-acute care team. In this role, you'll provide on-site physiatry and pain management services across skilled nursing and rehabilitation centers, working independently while partnering closely with interdisciplinary teams to help patients regain function, improve mobility, and manage pain effectively.

Key Responsibilities

• Perform comprehensive physiatry and rehabilitation evaluations and follow-up visits in SNF and rehab settings

• Diagnose and manage musculoskeletal, neurologic, and functional limitations common in post-acute patients

• Develop individualized treatment plans focused on functional restoration, mobility goals, and pain management

• Coordinate care with PT, OT, and speech therapy plans of care

• Monitor patient progress and adjust care plans as clinically appropriate

• Complete accurate and timely documentation of assessments and progress notes

• Communicate recommendations to facility clinical teams and support discharge planning as needed

Qualifications: Physician (MD/DO)

• MD or DO, Board Certified or Board Eligible in Physical Medicine & Rehabilitation (PM&R) preferred; candidates with strong pain management, MSK, or rehab medicine experience will also be considered

• Active New York State medical license

• Experience in post-acute care, rehabilitation, or pain management preferred

• Strong clinical judgment, documentation, and collaboration skills

Qualifications: Nurse Practitioner

• Adult-Gerontology NP preferred; FNP considered with relevant experience

• Active New York State NP license

• Experience in post-acute/SNF care, rehabilitation, pain management, geriatrics, orthopedics, or internal medicine preferred

• Ability to work autonomously, manage a patient panel, and collaborate with interdisciplinary teams

• Strong documentation and communication skills
