 Duties: 

• Develop, implement, evaluate and direct the social service programs of the facility

• Participate in community planning related to the interests of the facility and the services and needs of the resident and family

• Maintain community relationships to help address resident concerns as appropriate

• Perform routine tasks with patient admissions, care plans and discharge planning

• Coordinate response to reports of missing, lost or stolen belongings, along with other grievances 

Qualifications: 

• Must have, as a minimum, a bachelor’s degree in social work or a bachelor’s degree in a human services field including but not limited to sociology, special education, rehabilitation counseling, and psychology

• Social work experience in a skilled nursing/ long term care setting working directly with individuals

• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public

• Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
