Responsibilities:

• Provide direct patient care to residents, including administering medications, wound care, and treatments as ordered by physicians

• Monitor and report changes in residents' condition, including vital signs, lab results, and other relevant data

• Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident

• Educate residents, families, and caregivers on disease processes, treatment options, and self-care techniques

• Participate in quality improvement initiatives to ensure the highest standards of care and service

• Maintain accurate and detailed records of patient care and communication
