What You’ll Do:
• Conduct patient assessments & create care plans
• Perform OASIS assessments & monitor progress
• Coordinate care, referrals & discharge planning
• Work closely with patients and families

What You Need:
 Active CT RN License + CPR certification
 1+ year RN experience (home care a plus)
 Valid driver’s license & reliable vehicle
