About the Role:

We're seeking a certified and experienced Home Health Aide to provide personalized care and support to our patients. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to deliver exceptional care and promote independence, comfort, and well-being.

Responsibilities:

• Provide personal care assistance to patients, including bathing, dressing, grooming, and toileting

• Assist with daily living activities, such as meal preparation and medication management

• Support patients with mobility and ambulation, including transfers and exercises

• Monitor patients' health status and report changes to the supervising nurse

• Collaborate with the healthcare team to develop and implement individualized care plans

• Maintain accurate and confidential records of patient care and communication

Requirements:

• Current certification as a Certified Home Health Aide (CHHA)

• High school diploma or equivalent required

• Ability to lift 50 pounds and stand for long periods

• Strong communication and interpersonal skills

• Compassionate and patient-centered approach to care
