Responsibilities:

• Coordinate social service activities and programs to promote resident engagement and socialization
• Develop and implement individualized plans to meet the social service needs of our residents
• Provide emotional support and counseling to residents, families, and staff members
• Facilitate communication between residents, families, and healthcare providers to ensure optimal care
• Collaborate with interdisciplinary teams to address resident needs and concerns
• Maintain accurate and up-to-date records of resident social service activities and progress

Requirements:

• 0 minimum years of experience in social service coordination or a related field
• High school diploma or equivalent required; degree in social work, psychology, or a related field preferred
• Strong communication, interpersonal, and organizational skills
• Ability to maintain confidentiality and handle sensitive information
• Flexibility to adapt to changing priorities and needs
