The candidate MUST have:

• A valid driver’s license

• Valid driving documents

• Reliable transportation

• The ability to successfully pass all state-mandated background checks and screenings

Shift Details: Direct Support Professional (Caregiver)

• Monday: 8:30 AM–4:30 PM – Shared Support
• Tuesday: 8:30 AM–4:30 PM – Individual Support
• Wednesday: 8:30 AM–4:30 PM – Shared Support
• Thursday: 8:30 AM–4:30 PM – Individual Support
• Friday: 8:30 AM–4:30 PM – Shared Support

Total Schedule: Up to 40 hours per week

Core Responsibilities: Direct Support Professional (Caregiver)

• Personal Care: Assist consumers with daily routines and personal care needs as outlined in their individual service plans.

• Home Support: Assist with meal preparation, light housekeeping, and maintaining a safe and comfortable living environment.

• Daily Activities: Provide support with daily routines and activities while encouraging independence and personal choice.

• Community Support: Assist with community activities and appointments when required.

• Daily Documentation: Maintain accurate documentation of services provided and communicate any concerns or changes according to SLIC procedures.

Requirements: Direct Support Professional (Caregiver)

· Age: Must be 18 years of age or older.

· Documents: Valid Driver’s License or Valid ID and Social Security Card

· Background: Must pass all state-mandated background checks and screenings.

· Transport: Reliable transportation to consumer homes in Jennings may be required depending on specific position.

· Character: A patient, compassionate, and reliable work ethic.
