Wound Care Nurse (LVN)

We’re looking for a compassionate, detail-oriented Licensed Vocational Nurse (LVN) to join our team as a Wound Care Nurse!

This is a great opportunity for an LVN who enjoys wound care, skin integrity management, and providing specialized treatments in a Skilled Nursing Facility. You’ll work closely with nursing leadership, physicians, and the interdisciplinary care team to promote healing, prevent complications, and ensure residents receive high-quality, individualized care.

Responsibilities

• Perform and document physician-ordered treatments, including wound care, dressing changes, and skin treatments

• Assess, monitor, and document wounds, pressure injuries, and other skin conditions

• Collaborate with physicians and the interdisciplinary care team to implement individualized treatment plans

• Maintain accurate and timely documentation in residents’ medical records

• Monitor residents for signs of infection or complications and report changes in condition

• Follow infection control protocols and established wound care practices

• Participate in wound rounds and interdisciplinary care meetings

• Educate nursing staff on proper wound care techniques and skin breakdown prevention

• Assist with implementing interventions to prevent pressure injuries

• Maintain treatment supplies and ensure proper storage and infection control

• Ensure treatments are completed according to physician orders and facility schedules

Requirements

• Current, active LVN license in the state of California

• Long-term care, skilled nursing, or clinical nursing experience preferred

• Wound care, skin assessment, and treatment documentation experience preferred

• Knowledge of infection control practices and long-term care regulations

• Strong assessment, documentation, and organizational skills

• Ability to work independently and collaboratively with an interdisciplinary team
