Job Summary:

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team at Terraces of Lake Worth Care Center, a leading provider of Rehabilitation services in Lake Worth Beach, Florida. As a Per Diem PT, you will have the opportunity to work with a diverse group of patients and participate in providing high-quality care in a supportive and dynamic environment.

Responsibilities:

• Conduct physical therapy evaluations and develop individualized treatment plans for patients in our sub-acute and skilled nursing care settings

• Implement and modify treatment plans to address patient needs and goals, ensuring effective progress and recovery

• Collaborate with interdisciplinary teams, including nursing, occupational therapy, and speech therapy, to ensure comprehensive care

• Provide education and support to patients, families, and caregivers on physical therapy techniques and strategies

• Maintain accurate and timely records of patient progress and treatment plans

• Participate in quality improvement initiatives and contribute to the development of best practices in physical therapy
