SOCIAL WORKER

We are seeking a compassionate and motivated Social Worker to join our skilled nursing team. This position plays an important role in supporting our residents and families while promoting resident-centered care and quality of life.

RESPONSIBILITIES

• Complete social service assessments and documentation

• Participate in care planning and interdisciplinary meetings

• Assist residents and families with psychosocial needs

• Coordinate discharge planning and community resources

• Advocate for resident rights and preferences

• Maintain accurate and timely documentation

QUALIFICATIONS

• Degree in Social Work or related field as required by applicable regulations

• Minimum 1 year of Skilled Nursing/Long-Term Care experience preferred

• Strong communication and organizational skills

• Compassionate, professional, and team-oriented

• PointClickCare (PCC) experience preferred
