Job Summary:

Slate Health is seeking a skilled Physical Therapist to join our team in providing high-quality telehealth services to patients across the country. As a Physical Therapist Tele Health, you will be responsible for conducting virtual evaluations and developing treatment plans for patients in a remote setting. This is a part-time opportunity that can be based anywhere, with a requirement for licensure in Colorado.

Responsibilities:

• Conduct virtual evaluations and assessments to diagnose and treat patients with various physical therapy needs

• Develop and implement individualized treatment plans to achieve patient goals and outcomes

• Utilize electronic medical records and telehealth platforms to document patient information and communicate with patients

• Provide education and training to patients with proper techniques and exercises to promote overall wellness and rehabilitation

Requirements:

• Physical Therapist degree from an accredited program

• Current licensure in the state of Colorado

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Proficiency in electronic medical records and telehealth platforms
