Licensed Practical Nurse (LPN) Responsibilities:

· Admissions, Discharge and Transfers of residents

· Medication requisitions with pharmacy and physicians

· Medication administration

· Following infection control, emergency, and other center policies

· Collaborating with resident’s interdisciplinary team to provide the best outcomes

· Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy

Licensed Practical Nurse (LPN) Skills and Qualifications:

· LPN (Licensed Practical Nurse) licensed by the State

· CPR Certification required

· Two Years prior nursing experience preferred

· Valid NYS LPN license

· Previous experience in long-term care or a skilled nursing facility is preferred

· Strong communication and organizational skills

· Compassionate and dependable with a team-first attitude
