Home Care Registered Nurse

Synergy Homecare MSP Metro (office in Roseville) is seeking an experienced Registered Nurse to provide care in clients homes throughout the central metro area. This is a flexible part-time position (approximately 16-20 hours/week).

Duties:

·         Assess clients in their homes, create Plans of Care, and provide nursing services

·         Assist with caregiver training/education

·         Collaborate with clients, family members, care-providers, and co-workers to provide individualized client care

·         Provide reliable care and complete timely documentation of all visits and assessments

Qualifications:

·         Nursing degree, along with a valid MN registered nursing license

·         Strong clinical skills for assessing client needs and providing care

·         Communication skills

·         Critical thinking skills

·         Compassion and empathy

·         Customer service skills

·         Computer skills

·         Valid Driver’s license, insurance and a reliable vehicle
