Social worker

Axia Care Center is a long-term care and subacute Skilled Nursing Facility (SNF) committed to delivering high-quality, patient-centered care. We are seeking an experienced Social Worker to support our residents and families while collaborating closely with the interdisciplinary care team.

Key Responsibilities

• Conduct psychosocial assessments and develop individualized care plans

• Support residents and families with adjustment, discharge planning, and long-term care transitions

• Coordinate with nursing, admissions, therapy, and case management teams

• Ensure compliance with state and federal regulations

• Participate in care plan meetings, family meetings, and discharge planning conferences

• Maintain accurate and timely documentation in the electronic medical record

• Serve as an advocate for residents’ rights, dignity, and overall well-being

Qualifications

• Bachelor’s degree in Social Work (BSW) required; LSW or CSW preferred

• Active social work license or certification per state requirements

• Must have prior experience working in a Skilled Nursing Facility (SNF)

• Strong communication, organizational, and problem-solving skills

• Ability to function effectively in a fast-paced, team-oriented environment
