Description:
The Wound Care Director position is responsible for the effective direction, management, and operation of Wound Care programs.
Responsibilities:
- Evaluate wounds and collaborate with physician to ensure optimum delivery of care for wound healing.
- Record, measure and document resident's wounds/wound progress.
- Initiate nursing care procedures in managing acute, chronic and/or traumatic wounds.
- Track and monitor (for infection and healing) trending and record skin conditions and wounds/wound management.
- Coordinate treatment to provide comprehensive wound assessments and plan of care including goals, implementation of treatment, evaluation, and outcomes for patients throughout the facility.
- Provide patient, family and staff education regarding wound management and therapies.
- Act as the resource person for physicians and clinical staff for wound management.
- Perform general nursing duties as needed.
- Participate in quality improvement plan for the facility.
- Develops skin care programs to prevent skin breakdown.
- Demonstrate knowledge and skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care, and incontinence.
- Demonstrate revision to plan of care as indicated by the patient’s response to treatment and evaluate overall plan for effectiveness.
- Formulate a teaching plan based
Qualifications:
- Currently licensed as RN (Required)
- Wound Care Certified (Preferred)
- Minimum 3 years’ experience in working within a skilled nursing, short term rehabilitative environment.
- Prior experience with Wound Rounds (Preferred)
