Responsibilities
Provide direct patient care, including administering medications, performing vital signs, and monitoring resident conditions
Assist with activities of daily living (ADLs), such as bathing, dressing, and feeding
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Communicate effectively with residents, families, and healthcare staff to ensure seamless care coordination
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Document resident care and progress in accordance with facility policies and regulations
Requirements
Current licensure as a Licensed Practical Nurse (LPN) in the state of Nebraska
High school diploma or equivalent required; degree in nursing or related field preferred
Minimum 1 year of experience as an LPN; experience in a long-term care setting preferred
Strong communication, interpersonal, and teamwork skills
Ability to lift, transfer, and move patients with care and attention
Familiarity with electronic medical records and basic computer skills
