As a Licensed Practical Nurse (LPN) at Spring Creek Healthcare Center, you will play a vital role in delivering exceptional care to our residents. Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Monitor and report changes in patient condition to the Charge Nurse or Physician

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives and patient satisfaction surveys

• Collaborate with other healthcare professionals to ensure seamless care transitions
