Responsibilities:

• Provide skilled nursing care to ventilator-dependent patients in their homes, focusing on promoting independence, comfort, and overall well-being

• Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams

• Conduct thorough assessments, identify needs, and develop solutions to address patient concerns and barriers

• Administer medications, treatments, and therapies as prescribed by physicians, and monitor patient responses

• Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and coordination

• Participate in ongoing education and training to stay current with industry developments and best practices

• Maintain accurate records and reports, adhering to regulatory and compliance requirements

Requirements:

• Current RN licensure in the state of New York

• Experience working with ventilator-dependent patients and/or in home care settings

• Ability to work independently, with minimal supervision, and as part of a multidisciplinary team

• Excellent communication, interpersonal, and problem-solving skills

• Ability to adapt to changing patient needs and prioritize tasks effectively
