Birchwood Terrace Rehabilitation and Healthcare has an exciting opportunity for a Full-Time Physical Therapist (PT) in Burlington, Vermont!

We are looking for a dedicated PT to join our rehabilitation team of outstanding professionals who provide person-centered care to our residents and patients. Our goal is to help each individual achieve meaningful outcomes while maintaining the highest standards of quality and compassion.

Physical Therapist Responsibilities

• Consult with patients to understand their individual needs, concerns, and rehabilitation goals.

• Perform comprehensive patient evaluations and assess and interpret evaluation findings and test results.

• Develop individualized treatment plans using a variety of evidence-based physical therapy techniques and interventions.

• Create personalized rehabilitation programs designed to help patients achieve their functional goals and maximize independence.

• Administer medically prescribed physical therapy treatments to relieve pain, improve or restore function, increase mobility, and promote healing.

• Ensure all treatment is provided in accordance with the established plan of care.

• Monitor patient responses to treatment interventions and modify treatment approaches as clinically appropriate in collaboration with the interdisciplinary team (IDT).

• Provide clinical support, education, and guidance to patients, family members, and caregivers.

• Collaborate with other members of the interdisciplinary team to promote coordinated, high-quality patient care.

• Ensure all documentation is completed accurately, thoroughly, and in a timely manner in accordance with facility policies and applicable state and federal regulations.

• Comply with all applicable rules, regulations, policies, procedures, and professional standards.

• Attend required meetings and perform other duties as designated by the Director of Rehabilitation.

Physical Therapist Skills & Qualifications

• Previous experience in a skilled nursing facility (SNF) or long-term care setting preferred.

• Current knowledge of physical therapy treatment practices, techniques, and interventions.

• Familiarity with the Net Health electronic documentation system preferred.

• Strong clinical, communication, and interpersonal skills.

• Ability to work effectively with patients and residents with a variety of personalities, needs, and abilities.

• Strong organizational skills and attention to detail.

• Ability to collaborate effectively with patients, families, caregivers, and members of the interdisciplinary team.

• Commitment to providing compassionate, patient-centered care.

Physical Therapist Requirements

• Active and valid Physical Therapy license in Vermont.

• Must be in good standing with all applicable regulatory agencies and licensing boards.
