About the Position

Seagate Rehabilitation and Nursing Center is seeking a compassionate and reliable Home Health Aide (HHA) to escort residents to and from clinic and dialysis appointments while ensuring their safety, comfort, and well-being.

Responsibilities

• Escort residents safely to and from clinic and dialysis appointments.

• Arrive 1 hour prior to scheduled clinic appointments.

• Assist residents with preparing for appointments and provide out-on-pass snacks.

• Monitor residents for safety and help prevent falls or altercations.

• Report accidents, incidents, or concerns immediately to the Clinical Coordinator or ADNS.

• Ensure required paperwork and medical documentation accompany residents.

• Verify DNR/DNI status and required documentation.

• Advocate for residents and communicate clinic concerns to the facility.

• Maintain professional conduct and follow facility policies and abuse-prevention regulations.

• Coordinate with the Clinic Coordinator regarding assignments and appointments.

• May be required to attend multiple clinic visits in one day.

Qualifications

• Current NYS HHA certification required.

• CNA certification preferred.

• High school diploma or equivalent preferred.

• Minimum 1 year of EMR experience; Sigma experience preferred.

• Prior nursing home experience preferred.

• Professional appearance and strong communication skills required.
