Physical Therapist (PT) – PACE Program

About the Role:

We are seeking a passionate and experienced Physical Therapist (PT) to join a multidisciplinary care team at a reputable PACE (Program of All-Inclusive Care for the Elderly) facility in the Louisville Metro area. If you’re looking for a meaningful role where your clinical expertise directly improves mobility, safety, and quality of life for older adults, this could be a perfect fit.

Key Responsibilities:

  • Evaluate and assess participants’ physical function, mobility, gait, balance, and rehabilitation needs in both home and center environments.

  • Develop, implement, and modify individualized physical therapy care plans in collaboration with the interdisciplinary team.

  • Provide therapeutic interventions to help participants improve or maintain mobility, reduce fall risk, and enhance physical independence.

  • Instruct participants, caregivers, and family members in therapeutic exercises, mobility techniques, and use of assistive devices.

  • Document assessments, treatments, and progress in accordance with PACE program standards and regulatory guidelines.

  • Participate in care planning meetings and coordinate closely with RNs, PCPs, OTs, social workers, and other team members.

Qualifications:

  • Licensed Physical Therapist (PT) in the State of Indiana  (or eligible for licensure).

  • Minimum 1 year of experience in geriatrics, home health, SNF, outpatient, or PACE preferred.

  • Excellent interpersonal and communication skills; able to thrive in an interdisciplinary care model.

  • Proficient in EMR systems and documentation best practices.

  • Valid driver’s license and reliable transportation (home visits may be required).
