Salary: Starting at $80,000

 

Description:

The Wellness Nurse to monitor resident health, implement wellness plans, and coordinate with care teams to promote a safe and healthy environment.

 

Responsibilities:

•          Assess and monitor resident health, including vital signs and chronic conditions
•          Administer medications and treatments within scope of practice
•          Collaborate with physicians, nurses, and care teams on wellness plans
•          Educate residents and staff on health, nutrition, and preventive care
•          Maintain accurate health records and document interventions
•          Support resident engagement in wellness programs
  

Qualifications:

•          Active LPN or RN license in Illinois
•          Experience in assisted living, long-term care, or senior care (Required)
•          Knowledge of geriatric care and wellness principles 
•          Strong communication, organizational, and teamwork skills
