About the Role:

As a Licensed Practical Nurse (LPN) on our Per Diem team, you will provide high-quality, patient-centered care to our residents in a dynamic and supportive environment. Your responsibilities will include:

• Administering medications and treatments as prescribed by physicians

• Assisting with daily living activities such as bathing, dressing, and grooming

• Monitoring and reporting changes in residents' conditions

• Collaborating with other healthcare professionals to develop and implement individualized care plans

• Providing emotional support and compassion to residents and their families

Responsibilities:

• Provide direct patient care under the supervision of a licensed RN

• Document patient information accurately and thoroughly

• Participate in quality improvement initiatives to enhance resident care and satisfaction

• Maintain a safe and clean environment, adhering to infection control protocols and safety standards

• Communicate effectively with patients, families, and healthcare team members

Requirements:

• Current LPN licensure in the state of Wisconsin

• High school diploma or equivalent required

• Previous experience in a long-term care or acute care setting preferred
