• Collaborate with our interdisciplinary team to develop and implement personalized care plans

• Promote a positive and supportive environment, fostering a sense of community among residents

Responsibilities:

• Provide emotional support and companionship to residents

• Assist with light housekeeping, laundry, and meal preparation

• Apply wound care and perform physical therapy exercises as needed

• Monitor and report any changes in resident condition or behavior to the care team
