Elevate Care is looking for a compassionate Restorative Aide to join our dedicated team! If you are a Certified Nursing Assistant (CNA) with a passion for helping residents regain or maintain their highest level of function and independence, this is the role for you. You’ll play a vital role in providing restorative care and assisting residents in achieving their personal health goals.

Your Responsibilities:

Assist with Restorative Care Plans – Work with residents to implement individualized care plans focused on rehabilitation and functional independence.
Promote Mobility & Function – Help residents with exercises, mobility, and activities of daily living to improve strength and range of motion.
Track Resident Progress – Monitor and document residents’ improvement and progress in their restorative goals.
Provide Therapeutic Activities – Lead or assist in therapeutic exercises and activities to maintain or improve residents’ abilities.
Collaborate with Nursing & Therapy Teams – Ensure consistent, coordinated care for optimal results.
Maintain Accurate Records – Keep clear and up-to-date documentation of care provided and progress made.

Qualifications:

Active Certified Nursing Assistant (CNA) Certification
Previous Experience in Restorative Nursing or Long-Term Care (Preferred).
Strong Communication & Interpersonal Skills.
Ability to Work Independently & as Part of a Team.
A Compassionate, Patient-Centered Approach to Care.
Ability to Assist with Physical Activities and Mobility Tasks.
