Job Summary:

Irving Park Living and Rehab, a community nursing facility in Chicago, Illinois, is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) - PRN to join our team. About the Role:

• Provide direct patient care to residents, including administering medications, performing treatments, and monitoring vital signs

• Develop and implement individualized care plans in collaboration with interdisciplinary teams

• Collaborate with families and healthcare providers to ensure comprehensive care and support

• Participate in quality improvement initiatives to enhance the overall resident experience

• Maintain accurate and detailed records of patient care and treatment

Responsibilities:

• Deliver high-quality, patient-centered care to residents, including but not limited to:

  • Assisting with activities of daily living (ADLs), such as bathing, dressing, and grooming

  • Monitoring and managing chronic conditions, such as diabetes, hypertension, and COPD

  • Providing emotional support and psychosocial interventions

  • Collaborating with other healthcare professionals to coordinate care

• Maintain a safe and clean environment for residents, including but not limited to:

  • Ensuring compliance with infection control protocols and practices

  • Participating in sanitation and cleaning procedures

  • Identifying and reporting potential safety hazards

Requirements:

• Current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois

• Current CPR certification

• Ability to work variable shifts, including weekends and holidays

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
