We are currently seeking a compassionate and detail-oriented Care Plan/Restorative Nurse (LPN or RN) to join our skilled nursing team. In this specialized role, you will help promote resident independence, restore function, and improve quality of life through individualized care planning and restorative nursing interventions.

Your Role:

• Develop and implement individualized restorative nursing programs and resident care plans in collaboration with the interdisciplinary team

• Monitor resident progress and update care plans as needed to meet changing resident needs

• Collaborate with therapy staff to ensure continuity and consistency of care

• Provide hands-on restorative nursing care focused on mobility, ADLs, and rehabilitative support

• Promote resident independence, dignity, and functional improvement

• Ensure documentation is timely, accurate, and compliant with regulatory standards

• Educate residents and families on restorative goals and care techniques

• Support nursing staff in delivering resident-centered care that prioritizes recovery and well-being

• Perform additional duties as assigned in a patient-centered care environment

Who We’re Looking For:

• Current Illinois LPN or RN license

• Experience in long-term care or restorative nursing preferred

• Knowledge of MDS, care planning, and restorative nursing practices

• Strong communication, organization, and teamwork skills

• Passionate about helping residents achieve their highest level of function and independence

• Dependable, compassionate, and motivated team player
