• Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming, to our clients in their homes
• Assist with mobility and transferring clients, as needed, to promote comfort and independence
• Support clients with meal preparation, medication management, and other daily tasks
• Develop and maintain positive relationships with clients, families, and caregivers
• Collaborate with RNs, OTs, and other healthcare professionals to create personalized care plans
• Maintain accurate records and communicate effectively with colleagues and supervisors
