About the Role:

As a Licensed Social Services Director, you will be responsible for overseeing the daily operations of social services for Avir at Bellville and Avir at Sealy, ensuring that the facilities meet the highest standards of care and excellence. You will work closely with our interdisciplinary teams to develop and implement individualized care plans, provide emotional support and counseling, and ensure that our residents receive the resources they need to thrive.

Responsibilities:

• Divide time between Avir at Bellville and Avir at Sealy to lead and manage social services providing guidance and mentorship to ensure exceptional care and support.
• Develop, implement, and evaluate individualized care plans for residents, in collaboration with the interdisciplinary team
• Provide emotional support and counseling to residents, families, and staff
• Ensure compliance with regulations and standards, maintaining accurate and up-to-date records
• Collaborate with other departments to ensure seamless transitions and coordination of care
• Participate in quality improvement initiatives and team-building activities
• Maintain confidentiality and uphold organizational policies and procedures

Qualifications:

• Minimum of a bachelor's degree in Social Work or human services related field.

• Licensed per Texas state requirements or eligible for licensure.

• One year of experience in long-term care or other related medical care facility.

• Valid Driver’s license.
