Responsibilities:

• Provide skilled nursing assistance to patients in their homes, focusing on their unique needs and preferences

• Conduct vital signs, report changes to loved ones and healthcare providers, and document patient information accurately

• Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Demonstrate empathy and understanding in all interactions with patients and their families

• Collaborate with other healthcare professionals to develop and implement patient care plans

• Maintain a clean and safe environment for patients, adhering to infection control practices and other safety protocols

Requirements:

• Current CNA certification

• Reliable transportation and a valid driver's license
