Job Summary:

As a Licensed Practical Nurse (LPN) at Cornell Care & Rehabilitation Center managed by Autumn Lake Healthcare at Union, you will play a vital role in providing exceptional care to our patients. Our clinical excellence and compassionate care philosophy ensures that every patient receives personalized attention and treatment. We are seeking a dedicated and compassionate LPN to join our team of specialty caregivers.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and documenting patient information

• Develop and implement individualized patient care plans in collaboration with the interdisciplinary team

• Monitor and report patient progress to the healthcare team and families

• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care

• Provide emotional support and comfort to patients and their families

• Maintain accurate and up-to-date patient records

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
