We are seeking a dedicated and compassionate Evening Shift LPN to provide outstanding care to our patients and residents in Sunbury, PA.

About the Role:

As an Evening Shift LPN, you will play a vital role in delivering exceptional patient care and support to our residents. Your primary responsibilities will include:

• Providing direct patient care, including administering medications, wound care, and other treatments as needed

• Conducting vital sign checks, monitoring patient conditions, and reporting any changes or concerns to the healthcare team

• Assisting with mobility and ambulation, transferring patients, and providing support with daily living activities

• Maintaining accurate and detailed records of patient care and progress

• Collaborating with the interdisciplinary team to develop and implement individualized care plans

• Ensuring a clean and safe environment for patients and staff

• Participating in quality improvement initiatives and implementing changes as necessary

Requirements:

• Valid LPN license in the state of Pennsylvania

• Minimum 1-year experience as an LPN in a long-term care or rehabilitation setting

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment and prioritize tasks accordingly

• Commitment to delivering exceptional patient care and support

• Must be available to work evening shifts, rotating weekends and holidays
