About the Role:

As a Physical Therapist, PT, you will be responsible for providing high-quality physical therapy services to our patients and residents. Your expertise will be used to develop and implement individualized treatment plans to promote optimal physical function, alleviate pain, and improve overall well-being. You will also collaborate with our interprofessional team to ensure seamless care and achieve optimal outcomes.

Responsibilities:

• Provide physical therapy services to patients and residents, including evaluating, treating, and documenting their progress
• Develop and implement individualized treatment plans in accordance with patient goals and needs
• Collaborate with our multidisciplinary team to ensure comprehensive care and achieve optimal outcomes
• Conduct thorough assessments and screenings to identify physical therapy needs and develop effective treatment plans
• Implement evidence-based physical therapy interventions to promote optimal physical function and alleviate pain
• Educate patients, families, and caregivers on physical therapy techniques and strategies to support independence and wellness
• Maintain accurate and timely documentation of patient care and progress
