Job Summary:

We are seeking a compassionate and highly skilled Physical Therapist PT to join our team at Crest Haven Nursing and Rehab in Middle Township, New Jersey. Responsibilities:

• Conduct initial and ongoing assessments of patients to identify physical therapy needs and develop individualized treatment plans

• Provide physical therapy interventions, including but not limited to, exercises, stretches, and other manual therapies

• Collaborate with other healthcare professionals to develop and implement comprehensive treatment plans

• Communicate effectively with patients, families, and other healthcare professionals to ensure optimal care

• Maintain accurate and detailed records of patient treatments and progress

• Participate in in-service training and education to stay current with best practices and industry developments
