Responsibilities:

• Provide direct patient care, including administering medications, treatments, and procedures

• Develop and implement individualized care plans, in collaboration with the interdisciplinary team

• Conduct initial and ongoing assessments to identify guest needs and develop interventions to address those needs

• Collaborate with guests, families, and healthcare teams to ensure effective communication and care coordination

• Maintain accurate and detailed records of guest care and progress

• Participate in performance improvement activities, identifying areas for improvement and implementing changes as necessary

Requirements:

• Current licensure as an LPN/RN in the state of Ohio

• Valid CPR certification

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing competing demands

• Excellent communication and interpersonal skills, with the ability to work collaboratively as part of a team

• Strong problem-solving and critical thinking skills, with the ability to make sound judgments in challenging situations

• Ability to maintain confidentiality and adhere to HIPAA guidelines
