Pay Rate: $37.00/hr
Location: NYC 5 boroughs (report to Maspeth, NY)
Schedule: Full-Time, 10:00 AM - 8:00 PM (Variable Days)

Key Responsibilities:

• Provide home visits, monitoring, and treatment for adult and pediatric patients.
• Conduct assessments and treatment under APP supervision.
• Maintain accurate patient records via HealthPoint and Athena EMR.
• Engage in telemedicine support, adhering to clinical standards.
• Manage health, medication, and care goals for patients.
• Provide supervised treatment for common diagnoses.
• Participate in ongoing education and safety protocol adherence.

Qualifications:

• Minimum 2 years of experience as an LPN
• Current BLS certification (American Heart Association)
• Preferred experience in ED, Urgent Care, Critical Care, Family Practice
• Active NY State LPN license
• Pediatric clinical experience required
• Phlebotomy experience required
• Fluent in Spanish preferred
• Ability to lift 50-60 lbs
