Responsibilities:

• Provide one-on-one care to clients in their homes, including assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Support clients with mobility and transfer needs, using proper lifting and transfer techniques to prevent injury

• Assist with medication management, including administering prescribed medications and keeping accurate records

• Engage with clients in meaningful conversations and activities to promote socialization and mental stimulation

• Maintain accurate records of client care, including progress notes and medication administration records

• Collaborate with other care team members to ensure seamless care transitions and effective communication
