LPN Shift Supervisor

Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences) which has 210 beds, our independent living community (Fountaingate Gardens) and Gurwin Certified and Licensed Home Care Services Agencies. We are currently seeking a full-time LPN who will embody the passion and commitment required to deliver the ultimate care for our residents.

About this role:

$10,000 Sign On Bonus

Tuesday to Saturday - 3:00p.m. - 11:15p.m.

Requirements:

• Current LPN licensure in the state of New York

• Two years prior experience in long-term care or assisted living

• Strong clinical skills and knowledge of geriatric care

• Excellent communication and interpersonal skills

• Ability to work effectively as part of a team

Responsibilities include, but are not limited to:

• Supervision of trained Medication Aides

• Supervision and assignment of Resident Care Coordinators

• Supervision and maintenance of the 24 hour shift report

• Apprise family and M.D. of changes in resident’s health and behavior in the absence of the Director of Resident Services

• In an emergency, will pass medications to the residents

• Perform and verify NARC Count – Beginning and end of each shift

• Other related duties as directed
