• Conducting thorough and accurate assessments to identify residents' needs and develop effective care plans
• Collaborating with interdisciplinary teams to create and implement individualized plans of care
• Coordinating MDS assessments to ensure compliance with federal regulations
• Providing education and support to residents, families, and caregivers
• Maintaining accurate and thorough records of assessments, care plans, and resident progress
