Responsibilities:

• Provide high-quality care and assistance to residents in accordance with established care plans

• Monitor and report vital signs, changes in condition, and concerns to licensed nurses

• Assure a clean, safe, and comfortable environment for residents

• Support with activities of daily living (ADLs) such as bathing, dressing, and feeding

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Maintain accurate and timely records of resident care and services
